Spring 2025 Hospital Safety Grades Announced

56 Hospitals in Pennsylvania Earn an “A” on the Fall 2024 Leapfrog Hospital Safety Grade
22 Hospitals in Pennsylvania Able to Sustain “A” on the Leapfrog Hospital Safety Grade for Five of More Grading Rounds
The Leapfrog Group, an independent national nonprofit focused on patient safety and representing hundreds of the nation’s most influential employers and purchasers of health care, released the Spring 2025 Leapfrog Hospital Safety Grade. The biannual Safety Grade is an “A”, “B”, “C”, “D”, or “F” assigned to all general hospitals in the United States based on their ability to protect patients from medical errors, accidents, injuries, and infections. Driven in Pennsylvania by the Lehigh Valley Business Coalition on Healthcare (LVBCH), these grades provide a critical measure of patient safety for hospitals nationwide.
In Pennsylvania, 132 hospitals were graded, and 56 earned an “A,” 33 earned a “B,” 34 earned a “C,” 9 earned a “D,” and none earned an “F.” Overall, Pennsylvania has 42.4% “A” Hospitals and ranks as 10th in the country for percentage of “A” hospitals.
For the first time, Leapfrog is spotlighting the 346 hospitals nationwide that not only achieved an “A” for spring 2025 but sustained the grade for more than two years. This select group representing 12% of all eligible hospitals, is now highlighted on www.hospitalsafetygrade.org. In Pennsylvania, more that 16% of graded hospitals sustained this achievement.

- Geisinger Health System’s Bloomsburg, Lewistown* and Muncy Medical Center campuses
- Lehigh Valley Health Network’s Dickson City, Hecktown Oaks, Pocono, Schuylkill campuses
- Luke’s University Health Network’s Allentown*, Anderson*, Bethlehem*, Carbon, Easton*, Miners, Monroe*, Sacred Heart*, and Upper Bucks campuses. All 11 of St. Luke’s campuses, including the Geisinger St. Luke’s* campus in Orwigsburg, PA and the Warren* campus in Phillipsburg, NJ, earned an “A” safety grade.

“Preventable medical errors remain a significant challenge in hospitals across the nation, and far too many people are harmed or killed every year as a result,” said Amanda Greene, Director of Operations, LVBCH and Leapfrog Regional Leader. “We are proud of the hospitals in our community that earned an “A” grade and will continue to work with our community, our members, and our hospital leaders to make sure patients always come first.”

The Leapfrog Hospital Safety Grade is the only hospital ratings program focused exclusively on preventing medical errors and patient harm. It is updated biannually in the fall and spring, offering the public a reliable, independent assessment of hospital safety. The Grades are determined by public, peer-reviewed methodology, calculated by top patient safety experts under the guidance of a National Expert Panel.
