How to Apply for Membership
Applying for membership with LVBCH is simple.
Just complete the membership application form online.

and submit it via email to lvbch@lvbch.com
Once we receive your application,
we’ll send you an invoice for your annual membership dues.
Membership for Employers
Employer members are organizations whose employees can:
- Attend LVBCH events
- Participate in LVBCH committees
- Enroll in LVBCH programs (medical, dental, vision, prescription drug, etc.)
Membership for Associates
Associate members include healthcare providers, pharmaceutical companies, insurance providers, and benefits consultants or brokers. Associate members can:
- Enroll in LVBCH programs (medical, dental, vision, prescription drug, etc.)
- Attend selected LVBCH events
- Join selected LVBCH committees
⚠ Note: Associate members are not eligible to serve on the LVBCH Board of Directors.
