How to Apply for Membership

Applying for membership with LVBCH is simple.
Just complete the membership application form online.

Prefer to complete application by hand?
Download the Membership Application
and submit it via email to lvbch@lvbch.com

Once we receive your application,
we’ll send you an invoice for your annual membership dues.

Membership for Employers
Employer members are organizations whose employees can:

  • Attend LVBCH events
  • Participate in LVBCH committees
  • Enroll in LVBCH programs (medical, dental, vision, prescription drug, etc.)

Membership for Associates
Associate members include healthcare providers, pharmaceutical companies, insurance providers, and benefits consultants or brokers. Associate members can:

  • Enroll in LVBCH programs (medical, dental, vision, prescription drug, etc.)
  • Attend selected LVBCH events
  • Join selected LVBCH committees

⚠ Note: Associate members are not eligible to serve on the LVBCH Board of Directors.
